Physical Examination

Students registering for activities must have a current physical examination form submitted for the Athletic Directors' review before the student is authorized to participate in practice or tryouts. Physicals are valid for 12 months as of the date of the Physical Exam. Use Grade 6-8 Athletic Forms to register and use the Athletic Update Forms during the year.
